Many exchanges have competitive trading fees but then hit you on the way out with their withdrawal fees. I understand your concern. The highest "fixed fee" not being a percentage is charged by CoinsBank who charge a ridiculous 0.005 BTC when withdrawing BTC (regardless of how small the BTC-amount you are withdrawing is).
