he made a total of 9 withdrawals, including the last one that is pending (which was resolve now) and it totaled to BTC0.6863.

Right, it is almost certainly based off of total withdrawals within a fixed time period. If this wasnt the case, any exploiter could drain the hot wallet by simply withdrawing a medium sized amount over and over.
