So, we need a card reader to use this wallet? I'm not sure what is this and where someone would get one.

Yes, the website already mention you need USB 3.0 SIM/EID card reader which is turn-off since total costs to use this wallet is almost doubled.

And also I would need to use a custom, modified version of Electrum? 
Yes. At least for now.
They made a pull request on Electrum's repo for their plugin: https://github.com/spesmilo/electrum/pull/5533
