The striking example is the open source Linux utility that had for many, many years backdoor which made vulnerable encrypted SSH connections. Interesting that it is believed that no body in the world was suffered from this backdoor because of "bad actor sloppiness" otherwise "it would have been catastrophic for Linux users ^{me including}.
