I wasn’t that deep into it during those times and didn’t fully understand what was going on, but I’ve heard that this exploit caused real losses for some platforms. That’s probably why most casinos today require at least 1 confirmation before crediting your wallet.

I was equally not fully awear about it because as at then i was into fiat gambling and not crypto gambling. For the first question you asked, I have never heard or seen a casino that was hit so badly because of this, but I believe many casino will fall victim of this during those times. I have even searched for such kind of case but didn't get any. But if I come across such case I will communicate it here.
I think that the geniuness of cryptocurrency transaction made the casino back them to trust the system than fiat transaction, and never expected that their trust can be used against them.
