It depends on the setting if the provider support RTP variants, for example, Pragmatic and NoLimit City.
I've checked a few games on Duelbits for both providers, and there's no problem. I'd say the site is configured properly (disclaimer on), the demo RTP is exactly the same as the real RTP. But you should check it yourself.

The heart and whatever appear when the VIP system implemented, so if you rank up and haven't received the bonus, you should contact CS.
